Example: 1,000 USD at 0.92 with a 1.5 % fee gives 906.20 EUR — an effective 1.1035 USD per EUR; against a 0.93 mid-market rate the spread is 1.08 % and the whole exchange costs 2.56 %.

Conversion and fee
A rate is quoted as 1 unit of the base currency = rate units of the quote. Base → quote multiplies, quote → base divides. A fee can be a percentage of the amount, or a fixed charge in either currency; either way it is shown in both currencies and taken off what you receive.
Effective rate and spread
The effective rate is the amount you paid divided by what you received — the only rate that matters to you. If you also enter a mid-market (reference) rate, the spread is how far the offered rate sits from it, and the total cost is what you would have received at mid-market minus what you actually get, fee included.
No rates are fetched
Rates move minute by minute and differ between counterparties, and this site fetches nothing; every number here is one you typed from your own quote. The round trip shows what converting back at the same rate and fee would leave — a way to see the cost of a there-and-back. Nothing leaves the browser; the same four anonymous usage counts as the rest of the site apply.
